TIME : 3-5PM
LOCATION : GOODYEAR RECREATION CENTER 420 ESTRELLA PARKWAY,
GOODYEAR, AZ 85338
PRICING: ONLINE PRE-REGISTRATION: $50
DAY OF REGISTRATION: $75
Athlete MUST bring*:
Proof of current medical Insurance (copy of insurance card). Insurance should be under your parents, yourself, or your spouse through another plan.
Provide a copy of a valid current Athletic Physical, or Doctor Dictation from the visit. (Unless a significant injury, a Physical Form for the 2023 Football Season should suffice.)
Signed copy of the Fitness Declaration, Waiver and Release of Liability (if under 18 must also be signed by parents).
Athlete is responsible for their own work-out gear,
including cleats.
